Continuous Glucose Monitoring (CGM) & Insulin Pump Services

Our office offers Continuous Glucose Monitoring (CGM) and insulin pump support services to help optimize diabetes management through detailed glucose data and device insights. These tools allow for more precise and individualized treatment plans.

We work with commonly used devices, including:

  • Dexcom (G6, G7)

  • Freestyle Libre (Libre 2, Libre 3)

  • Select insulin pump systems (based on provider recommendation and insurance coverage)

Services We Provide:

  • CGM placement and setup (if applicable)

  • Review and interpretation of CGM and insulin pump data

  • Treatment plan adjustments based on glucose trends and reports

  • Coordination with device representatives for training and setup

Please Note: Device training and initial setup for CGM systems and insulin pumps are provided by manufacturer representatives. Our office will coordinate these training sessions with you and the device company as needed.

Insurance & Billing

CGM and insulin pump–related services (including data interpretation and medical management) are billed to your insurance as medical services. Coverage varies by plan and is not guaranteed.

Patients are responsible for verifying:

  • Coverage and benefits

  • Copayments, coinsurance, and deductibles

  • Prior authorization requirements

Any portion not covered by insurance will be the patient’s responsibility.

Patient Acknowledgment & Financial Responsibility

By proceeding with CGM or insulin pump-related services, you acknowledge and agree that:

  • These services are part of your provider-directed medical care;

  • Insurance coverage and payment are not guaranteed; and

  • You are financially responsible for any balance not covered by your insurance.

Important Information

  • Some CGM and insulin pump services may require prior authorization

  • Training is completed through device representatives, not directly by our office

  • Insurance processing times may delay final billing

  • Follow-up visits may be required for ongoing data review and treatment adjustments

  • Patients should contact our office in advance with any questions regarding services or insurance coverage
