Medication Refills
Medication refill requests are processed during regular business hours, Monday through Friday. Refill requests must be submitted through the pharmacy or patient portal. Please allow up to 72 business hours for processing.
Important guidelines:
Only medications prescribed by our providers will be refilled
Refills are not processed on weekends or holidays
Controlled substances require regular follow-up visits
Patients must be up-to-date with scheduled appointments to ensure safe continuation of therapy
Missed or delayed appointments may result in limited refill quantities until the patient is seen
For patients using mail-order pharmacies, regular follow-up visits are required at least every three months.
Patients are encouraged to request refills in advance to avoid interruptions in medication therapy.
Prior Authorizations
Prior authorizations are completed only for medications that are determined to be medically necessary.
Insurance may require step therapy or covered alternatives prior to approval
If a provider-recommended alternative is appropriate, it may be substituted
If a patient requests a medication that is not deemed medically necessary by the provider, a $75 administrative fee will apply for completion of the prior authorization request
Approval is not guaranteed and is determined by the insurance company
Patients are responsible for providing any required documentation, including:
Lab results
Imaging or diagnostic reports
Documentation of previously tried and failed medications and reactions
Prior authorization requests are submitted within 72 business hours of receipt and may take up to 5 business days for insurance review and response.
Pharmacy Information & Refills
Requests must be submitted through your pharmacy whenever possible
Only medications prescribed by our providers will be refilled by this office
Patients are responsible for ensuring their pharmacy information is accurate and up to date
If you change pharmacies, please notify the office prior to submitting refill requests
Patients must be seen regularly to continue medication management safely
Refills may be limited if appointments are missed or not kept current
Controlled substances require routine follow-up visits per provider discretion
The office reserves the right to withhold refills if clinical follow-up is not maintained
