Current insurance card
Valid photo ID, e.g. driver’s license
Visa or Mastercard to secure on file
Cash, Visa or Mastercard for office co-pay due at time of service
Recent lab/radiology results from your referring doctor
Valid insurance referral (if your plan requires one)

All follow-up appointments will be in the office.
If you are an existing patient and need to send lab results or glucose logs, you can upload them to the Contact form and the documents will be forwarded to the doctor. Please note that the form is sent to a secure e-mail viewable only by the Office Manager and the lead medical assistant. Please do not send any medical questions that you need answered by the doctor through this form.
